Agencies, associations and individuals can sponsor (host) a workshop for Continuing Education Units. Each workshop is approved on a case-by-case basis. To apply for CEU's fill out the application and send back 15 days prior to the workshop with all required supporting documentation.

CEU Workshop Approval Application - Must be signed by sponsor and returned 15 days prior to the workshop.

Supplemental Form CEUINST - Must be used to list additional instructors that will be making workshop presentations. You can make as many copies as necessary.

Supplemental Form CEUML - Must be used to list additional locations if workshop will be held at multiple locations. You can make as many copies as necessary.
