The Dairy Division offers Certificates of Origin and Free Sale and Health Certificates as a service to the plants that we inspect.  These certificates assist these companies in exporting their products out of the United States.

If a plant or distributor needs a certificate please do the following:

• Download and fill out the sample Certificate of Origin (PDF) and Health Certificate (PDF) as completely as possible, you may fill out the form online
• Print out the completed form, or you may email it by clicking on the "Submit by email" button
• Fax or mail the completed certificates to the Dairy Division office:

• Include any special instructions

• Include a Federal Express Account Number to charge the cost of shipping the Certificates to the required location
• Please allow as much time as possible to process these certificates

Once received turn around time is typically three to five days.
